I don't get why people with tourrettes swear. Why don't they say other things? Why do so many of them have the tendency to say fuck, shit, etc... it just doesn't seem realistic to me.

Not all swear. At most, about 15% do. More often than not uncontrollable outbursts of swearing or profanity are symptoms of Coprolalia. Although similar, it is not the same as Tourettes.
