Alcohol would fit your description then; you can have severe seizures or even die from alcohol withdrawal. It's not hard to argue alcohol is the most serious physical dependency, barbiturate withdrawal can be deadly to, and Benzo withdrawl can cause seizures, but a full fledged heroin addict might wish he was dead, but he won't actually die from withdrawal, the full fledged alcoholic might; they will almost certainly go through terrible physical symptoms.

People do use it differently than other drugs due to the way it fits into society, but when people get drunk, it's undeniably a drug.
